TOWN OF CATHLAMET

REQUEST FOR QUALIFICATIONS AND

PROPOSAL (RFQ/P) FOR

ENGINEERING FIRM

FOR WATER SYSTEM PLAN UPDATE

The Town of Cathlamet will prepare its six-year update of the Water System Plan (WSP) that complies with WAC 246-290-100, in accordance with Washington State Department of Health (DOH) requirements. The purpose of this RFQ/P is to select a qualified engineering firm to prepare the Water System Plan.

SCHEDULE:

The draft WSP report shall be completed and submitted to DOH by June 30, 2014. A response to DOH comments and incorporation of DOH comments and recommendations (if any) into the WSP shall be made within the timeframe established by DOH. DOH must give final approval of the WSP by December 31, 2014. The time of final adoption of the WSP by the Town will depend upon the scope and detail of DOH comments and recommendations.

RESPONSE: Please provide five (5) copies of the following information:

1. Statement of Qualifications, including involve

ment in similar projects and references, for

each professional participating in the project.

2. A description of the methodology proposed for

use in response to each of the tasks outlined in

the above scope of work.

Please limit the response to 20 pages, excluding any attachments. The selection process will focus on the qualifications and experience of personnel in providing similar services, and on the relevance and of the proposed approach to address the Town’s water plan needs.

DUE DATE: The five (5) copies of the RFQ/P response must be received no later than 4:00 p.m., Monday, December 2, 2013, addressed or delivered to:
